Hockey Canada requires that all associations receive a copy or visual verification of every player's birth certificate.

Here are a couple of options for you:

1.  Online - a direct submission.
Click here:  BIRTH CERTIFICATES (Just take a pic with your phone or upload a document)
Note:  If uploading a pic from your phone please make sure to shrink it prior to uploading.  File size cannot be larger than 2MB

2.  Email a copy of the Birth Certificate to:  [email protected]
Please include player's last name and Birth Certificate in subject line.
3.  Fax a copy to our office

4.  Drop a copy off in our mail slot
