REFUND REQUEST FORM (Richmond Hill Hockey)

No refunds will be issued after October 01, 2019. From May 15,2019 until June 15,2019 refunds are subject to an admin fee of $150. From June 16,2019 until October 1,2019 refunds are subject to an admin fee of $250. Admin fees are per player. All refund requests are subject to approval by the Richmond Hill Hockey Association Board of Directors. Please allow a minimum of 2 weeks to receive a refund. Refunds will not be considered if this form has not been submitted.

IMPORTANT: Original credit card and/or debit cards used for registration must be used for any refunds.
  1. If payment was made on-line through HCR then your refund can be processed electronically. If payment was made manually by credit card at the hockey office then you have the option of calling in your credit card number (905-770-0550); faxing (905-770-7712) or coming into the office for the refund. If payment was made using your debit card then you must come into the office for your refund. Please remember to bring your original card with you.
